How do you tell the difference .....Does anyone know how to tell the difference between a physical ailment being meant to be, as in part of ones path or because of an emotional/mental problem manifesting as a physical illness?

Ok, I had to read this a few times....I "think" I do.
The first lady to come into the shop asking about Reiki wanted it cuz her back hurt, but I could see clearly it was her heart, a grief that was causing the hurt...carrying the load of responsibility for her mom and family since her dad died. Do you mean it this way?
We did fix it all the way round and she did need it to learn to deal with the loss and to know it was not her job to bear all their grief on her shoulders.
There's been a few others..not lots...not sure if this helps Mia dear....let us know.

Aaah, you 'see' I have not learnt or not got the gift to do that :(
Maybe I have tho, cos I think the one I know is suffering because of emotional problems.
I am too close to help tho, so I shall ask those above to help him.

I am not sure if it is a gift, it is more my guides letting me know this is more than just a Reiki session. Emotions can manifest illness and pain hugely. I can look at a pic if you wish it.

I think if you can read a person's chakras, that may help too.
I can't "see" the chakras or the colors in auras like some are able to. But when I do Reiki in person, I can usually "feel" a thickness where work needs to be done.
